Prevention


Overview

As part of the Medicare 9th SOW, Qualidigm will collaborate with primary care physician office practices to improve the quality and frequency of preventive health care services in order to optimize beneficiary quality of life and promote more efficient and effective utilization of healthcare services. The Prevention Theme focuses on the following services:

  • Breast Cancer Screening
  • Colorectal Cancer (CRC) Screening
  • Influenza Immunization
  • Pneumococcal Immunization

Qualidigm is working with selected physician practices in Connecticut to implement strategies to improve rates for the identified preventive services. These practices must have implemented an electronic health record (EHR) certified by an entity recognized by the Secretary of Health and Human Services, such as the Certification Commission for Health Information Technology (CCHIT).

Qualidigm Activities

Qualidigm interventions that support health information technology (HIT) are designed to improve screening and immunization rates by establishing systems that notify providers and patients they should be scheduled. The following services will be offered to the participating practices:

    • Consultative and technical assistance to each practice to optimize utilization of the certified EHR including identification of patients due for screenings and immunizations
    • A variety of educational opportunities for each participating practice on using its EHR capabilities to improve screening rates and immunization rates
